The Role of Teenage Living Donor in Korean Transplantation Activity

Introduction: In contrast to the Western country, living donation is relatively more common in Asia. For example, living donor liver transplantation comprises > 90% of liver transplantation activity in Asia, while in Europe and USA it comprises < 5% (1).  In Korea, the legal age of consent for organ donation is 20yr, but it can be lowered to 16 yr when the recipient is the donor’s parent. As yet the role of teenage donor in Korea transplantation activity has not been analyzed. In this study, we retrospectively analyzed the statistical features of Korean teenage donors.
Method: The statistical data service system of Korea Network for Organ Sharing was used. (https://www.konos.go.kr/konosis/common/).  From the KONOS data service system, demographic data of liver and kidney transplantation donor during the past 10 years (2007-2016) were retrieved and analyzed. The proportion of teenage donor among the whole donor population and the gender distribution were analyzed.
Results: A total of 11,609 liver transplantation and 15,719 kidney transplantation were performed in Korea during the past 10 years. The proportion of living donor in Korea is significantly larger than deceased donor; consisting 72.4 % of liver transplantation and 57.4% of kidney transplantation.  The contribution of living teenage donor to Korean transplantation activity during the past 10 years is shown on Fig 1. The teenage donors contribute 5.1-7.5% of the living donor, and 3.0-5.3% of the whole donor population including deceased donor. Fig 2 shows the gender distribution of teenage donors. Among the teenage donor, th eprportion of  male gender was larger than female, and the trend was maintained  throughout the 10 years.
Discussion and conclusion: Living teenage donors account for 5-7% of the whole living donor population in Korea. This is a sharp contrast with American data of the 0% living donor among age <18 in USA (2). The reason of skewed gender distribution - the absolute male dominancy- in this age group is not clear. This phenomenon may be related to the recent trend in Korea, which has a strong emphasis on girls ' appearance(3). Further investigation for the reason of gender skew among Korean teenage donors should be followed.
